Completing the TSR (Telephone Service Request) Form
The Telephone Service Request is a one-page form used to request additional telephones, adds, moves, and changes, and requests for installation of data network lines. This form is now available online in PDF so you may print it out. Telecom requests a signed original form or signed fax due to the need of an authorized signature.
The TSR form has three sections. Section 1 requests all account information and signatures. Please fill in your name as the "Requestor" on the first line, with the department noted on the second line. A completion date is required. Telecommunications requests an actual date, that is, something other than "ASAP" to be filled in here. Departments should allow 5 to 10 working days for completion of any request. The index code is necessary, and more than one code may be indicated if charges need to be allocated to more than one budget. The final signature is reserved for the chair, dean, or director. In some cases, the person having signature authority for the index code being charged is acceptable.
Section 2 contains the instructions for completing the TSR. It outlines information needed in Section 1 and notes that a complete description of the project should be defined in Section 3.
Please include a floor plan with every submission (except equipment requests). This "floor plan" can be a copy of an existing layout which most departments keep on hand, or it can be as simple as a pencil drawing noting where desks are in relation to the doors and windows. If you are requesting an installation, please mark the location where you would like it. If you are requesting an install or activation at an existing jack, please include the ID number of that jack. Please make a copy of the TSR. It is for your records and often comes in handy for reference.
Section 3 requests a description of the work. This can be as simple or elaborate as desired. If it does contain more information than can fit in the section, feel free to write on the back of the form or attach an additional page to the TSR. If the telephone counselor (or Requestor) has a question regarding the information to be provided in this area, he or she is welcome to call the Telecommunications System Administrator at 2-6300.
If you are requesting an estimate before the work is started, check the box "Estimate" located at the top of the form. The form will be filled in with the completed estimate and faxed back to you for approval before the work is implemented.
To request changes in offices for faculty and staff, send an email to dunaway@sou.edu. A standardized excel form for this purpose will be emailed back to you.
