Monday's          9:00 a.m. - 5:00 p.m.    

Tuesday's         8:00 a.m. - 5:00 p.m.  

Wednesday's    8:00 a.m. - 5:00 p.m.

Thursday's        8:00 a.m. - 5:00 p.m.

Friday's             8:00 a.m. - 5:00 p.m.

Medical and mental health services are available in alignment with the academic calendar. 

The center is closed for clinical services during winter break, spring break, and from the end of summer session until the start of fall term.  Part-time hours of service are provided during summer sessions.

 

The clinic is open for administrative and program questions 12 months of the year.

 

If you need ADMINISTRATIVE ASSISTANCE during the times the center is closed for clinical services,  please call Susan Bash, at (541) 552-6692.  Administrative staff are available Monday - Friday throughout the calendar year and can be contacted by phone, email or at the clinic (please ring the door bell when doors are locked).

 

Scheduling

The SHWC provides care on an appointment basis. Students without an appointment may have to wait to be seen.  Students with an appointment will be seen first.  Walk-ins will be accomodated in the order in which they arrive.

A triage nurse is available to assess medical necessity for immediate care.

You will lose your appointment if you show up past the time you were told to check in and your account will be charged a $20 no-show fee.

At the time of your first appointment, you will be asked to fill out a Comprehensive Health History form and an Informed Consent form. These forms are also available on the website. If you choose, you may fill them out in advance and bring them to your first appointment.

 

ELIGIBILITY

Health Fee
All students who are enrolled for 6 or more credits automatically pay a per term non-refundable health fee. In academic year 2009-10 the health fee is $111. Payment of the health fee makes students eligible for primary medical care, mental health/personal counseling and health education services. Students taking less than 6 credit hours have the option of paying the health fee in order to receive services.

 

Measles
Oregon State law requires students enrolled in 9 or more credit hours to provide proof of immunization to measles, except as permitted by religious or medical exemption. The SHWC requires this information from you and is authorized to place a hold on your account preventing you from registering for a second term until the information is provided.
