Below you will find a list of forms needed for class registration, proof of residency, application for a degree from SOU, application to SOU as a non-admitted or returning student and more.  A brief description of each form follows each listing.

• Transcript Request Form
  Use this form to request your SOU transcripts.
• Class Registration Form
  Use this form to register for classes when not doing so on-line.
• Add/Drop Slip
  Use this form to add or drop classes.
• FERPA Release
  Use this form to consent to the release of your educational records, to parents, for example.
• Non-Admitted Student Application
  Use this form when applying to the University for non-admitted status.
• Returning Student Application
  Use this form when returning to the University from an absence of three terms or more (not including summer term).
• Probation Mid-Term Report
  Use this form obtain your instructors' evaluation of your mid-term progress when on academic probation.
