Forms Used During The Application Process
If you do not have Acrobat Reader to download pdf files, please see below.
- You may apply for Undergraduate or Graduate Admission to Southern Oregon University using our Secure On-Line Server. The server may be unavailable during certain regularly scheduled maintenance periods.
- You may also print out a paper application (pdf) and mail your completed application with the $50 application fee to us: Admissions Office, Southern Oregon University, 1250 Siskiyou Blvd., Ashland, OR 97520. Here are the various forms:
- You may qualify for a deferral of the $50 application fee. If you qualify, the application fee would not become due until you register for classes. Do not apply on-line. Fees paid on-line cannot be refunded. To see if you qualify, complete the deferral form and send it to us with your paper application.
- If you were High School Class of 1997 or later, you will need to show completion of 2 years of the same high school level second language or 2 terms of the same college level second language. If you are unable to demonstrate this proficiency, we can admit you with a Second Language Contract (pdf), which would instead require you to complete the foreign language requirement prior to graduating from SOU.
- If you are an International Student, there are several other forms that you need to fill out. Please see the International Student pages for more details.
- Application for Foreign Student Admission (pdf)
- Report on Proficiency in English (pdf) and
- Financial Certificate/Visa Information Form (pdf)
- If you are currently attending school in the United States, you may also need to complete an International Student Advisor Report (pdf).
- If you need to change the quarter you wish to enter the University in a different quarter than the quarter you originally applied for, you will need to complete the Quarter Change Application.
- If you want to apply for financial aid and scholarships, please go to our Financial Aid web site where you may use our on-line application once you have completed your FAFSA.
